About Us
Diane Kelly, RN, PHN, MS, CMC, Chaplain
Diane is a trained Nurse Gerontologist since 2000 and has her own professional geriatric care management business Desert Geriatric Services where she assist families and friends with planning and caring for their senior or disabled loved ones.. She is not only a strong advocate for the elderly, but promotes improving how we are living until we die, along with development of good palliative care practices in health care systems. She now functions as a palliative care expert, along with founding the non-profit organization Partnership for End of Life Choices. Her volunteerism includes volunteering for the City of La Quinta’s Emergency Response Team, La Quinta Senior Center where she volunteers her Professional Care Manager knowledge twice a month. At Eisenhower Medical Center she is a Volunteer Lay Chaplain where she has helped develop their Twilight Vigil Program, "No One Dies Alone". She serves as a Board Member for Parkinson’s Resource Organization, along with being President of Oncology Nurses Organization Desert Chapter, a trained Faith Community Nurse, and a Rotarian.
Diane received her Master’s degree in Nursing Administration and certification in Applied Gerontology from California State Los Angeles in 1989, and worked for Kaiser Foundation Hospital from 1982 to 2003 where she managed their Home Health, Hospice and Skilled Nursing Facility Services.
Diane has presented many workshops on advanced care planning and giving, end of life decisions, living wills through Southern California and Coachella Valley. She also functions as a Certified Mediator for Riverside County and provides family mediation services.
Vision Statement
My vision is that the need for healthcare services will be a smooth transition from home to hospital or any other care setting with the use of a care manager.
